Returned checks will incur a fee as allowed by state law. Any collection costs incurred due to non-payment will be the responsibility of the patient or responsible party.

Insurance: As a courtesy, we will file insurance claims on your behalf. However, your insurance policy is a contract between you and your insurance company. Cancellation Policy: We require 48 hours’ notice for routine appointment cancellations and 72 hours for longer appointments or specialized procedures. Failure to provide adequate notice or missing appointments may result in a cancellation fee. Repeated cancellations or no-shows may result in discharge from the practice.

Financial Arrangements: For extensive treatment plans, we offer various payment options including third-party financing through CareCredit and Cherry. Financing approval and terms are determined by these companies, not by Church Dentistry. We also offer in-house payment plans for qualified patients, subject to approval and terms.

Privacy and Data Protection

HIPAA Compliance: Church Dentistry is committed to protecting your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). Our Notice of Privacy Practices, available separately, details how we may use and disclose your protected health information.
